FOR YOUR SAFETY: You shouldn’t have CryoSkin treatment if you have any of the following medical conditions. If you are unsure, please seek medical advice from your GP.
Severe allergy to colds, progressive diseases (MS, ALS, Parkinson’s, Neuropathy), active cancer, lymphatic disorders, uncontrolled diabetes or related complications, severe kidney or liver disease, pregnancy/breastfeeding, bacterial and viral infections of the skin, wound healing disorders, circulatory disorders, surgery in the past six months, pacemaker/metal implants, active eczema, rashes or dermatitis, use of topical antibiotics in the desired treatment area, mesh inserts in the desired treatment area, irremovable piercings, incision scar in the desired treatment area, silicone/other implants in the desired treatment area.

WAIVER Of LIABILITY and HOLD HARMLESS AGREEMENT

In consideration for using any Your Body Revolution services, including Cryoskin services, I at this moment RELEASE WAIVE, DISCHARGE IN ADVANCE, and HOLD HARMLESS YOUR BODY REVOLUTION, along with its EMPLOYEES from any. All liability, claims, demands, and actions arising from or related to any damage or injury that may be sustained by me while undergoing treatment or due to the use of equipment. I, at this moment, confirm that no warranty or guarantee, or other assurance has been made to me covering the results of any of the treatments/services that Your Body Revolution offers me. I have been explained and understand the administration of the process of my chosen treatment, including the FOR YOUR SAFETY section and other possible complications. I know that I am giving this consent voluntarily to use the equipment. I further, at this moment, AGREE TO INDEMNIFY AND HOLD HARMLESS YOUR BODY REVOLUTION from any costs that may incur due to the use of equipment by me. WAIVER of LIABILITY and HOLD HARMLESS AGREEMENT shall be construed by the laws of Queensland. I understand and have been advised that if I suffer from any of the above medical conditions or illnesses, I AM NOT TO HAVE CRYOSKIN TREATMENT. Agreeing to these terms constitutes my acknowledgment that I have read, fully understand and agree to the preceding CONSENT to the proposed Massage, Cryoskin, Compression Therapy, or any other treatment/service offered by Your Body Revolution. The process has been satisfactorily explained to me, and I have all the information I desire. I, at this moment, give authorisation consent. This CONSENT shall stand as long as I am in treatment at the location now and in the future. Agreeing to the terms, you will be subscribed to our mailing list and can unsubscribe anytime. IN TICKING YES TO THIS RELEASE, I ACKNOWLEDGE AD REPRESENT THAT I have read and understand the preceding WAIVER of LIABILITY and HOLD HARMLESS AGREEMENT and that I am fully competent.
